Marriage Certificate for a Name Change

If you are changing your name due to marriage, you will need to send a certified copy of your marriage certificate or your actual marriage certificate. We highly suggest sending a certified copy, since on very rare occasions documents have been lost, and you will probably want your original marriage certificate for personal reasons.

You can get a certified copy by send a letter to the office of vital statistics for your state. The cost varies by state, but it's generally about $15. Congratulations on your marriage!
