Can I apply for a renewal by mail if I lost my passport?

Renewing a passport by mail requires you submit your most previously-issued passport. If your passport was lost and you cannot submit it then you cannot apply for a renewal by mail. In such a case, it is necessary to apply for a new passport at an application acceptance facility. Before doing this, call the National Passport Information Center at 1-877-487-2778 to report the lost passport if it has not expired. The next step is to locate the nearest application acceptance facility and call to make an appointment. There are more than eight thousand facilities in the United States and its territories. Take a completed, unsigned Form DS-11 and required supporting documents to the facility. An authorized agent will check your documents, administer an oath and witness as you sign the application form. The form, supporting documents and payment will be placed in an envelope and sealed. You need to address the envelope according to the processing time you select. The options are standard service in 4-6 weeks or expedited service in 2-3 weeks. Addresses are located on the application form. Faster service requires you make an appointment and visit the nearest regional passport agency. An alternative for those unable to visit a processing center is to utilize the services of a registered expediter such as Fastport Passport.
