To renew a U.S. passport by mail, the document you need include: ✔ Your most recent, undamaged passport issued within the last 15 years. ✔ A recent passport photo (taken within the last 6 months) ✔ Evidence of name change IF your name was legally changes since your most recent passport was issued. Mail these documents along with the completed Form DS-82 and payment for the renewal fee to the address on the application form.
