A passport issued to a minor age 16 or younger cannot be renewed by mail using form DS-82. Rather, it is necessary for the child to appear with both parents at an application acceptance facility in order to apply for a new passport. When the minor appears with only one parent, the applying parent must submit either evidence of sole legal custody or notarized consent from the non-applying parent. Required documents include the completed application Form DS-11, evidence of U.S. citizenship, proof of child/parent relationship, ID of parent(s), photocopy of identification document(s) and passport photo. The application can be submitted at an application acceptance facility for routine or expedited processing service. Those who need to get a child's passport in less than two weeks can schedule an appointment at a regional agency or employ the services of a registered passport and visa courier.Source: http://www.us-passport-service-guide.com/child-passport.html

The steps to renew an expired passport depend on the date of issuance, the condition of the passport and how old you were when it was issued. If you were age 15 or younger when the expired passport was issued, it cannot be renewed by mail. If you were age 16 or older when the expired passport was issued but the date of issuance is more than 15 years ago, it cannot be renewed by mail. If the expired passport has been significantly damaged, it cannot be renewed by mail. In all the cases cited above, the applicant must appear at an application acceptance facility and apply for a new passport by submitting Form DS-11 and all supporting documents. If you were age 16 or older when you were issued the passport, it is not significantly damaged and it expired less than 5 years ago; you can apply for a passport renewal by mail.Source: http://www.us-passport-service-guide.com/expired-passport.html

In order to renew a U.S. passport by mail, the passport cannot be significantly damaged. Significant damage does not refer to normal wear and tear. It is perfectly normal for page corners to get bent, the emblem on the front cover to become faded and inside pages to get a little dirty from handling. Things that can lead to invalidation of a passport include significant tears or holes in the travel document, especially the cover or data page. Unoffical markings on the data page can also get it invalidated. You can apply for a passport renewal by mail if it has only normal wear and tear. If it has significant damage, you need to visit an application acceptance facility in order to apply for a new one. We recommend that applicants who are uncertain about the degree of damage, apply for a new passport. If you try to apply for a renewal with Form DS-82 and your passport is determined to be significanly damaged, you could be asked to apply again using Form DS-11 and required to pay additional fees.Source: http://www.us-passport-service-guide.com/damaged-passport-replacement.html

Many countries require a certain number of blank pages in a passport before they will issue a visa. If your passport only has 2-4 blank pages remaining and you need to obtain muliple visas, you may have a problem. There are a couple of ways you can remedy this depending on the expiration date of your passport and hwo frequently you travel. One, you can apply to add pages to your passport. All you need to do is mail the completed Form DS-4085 along with your current, valid U.S. passport and payment of $82 to the address on the form. You can choose to have 48 visa pages added to your passport instead of the standard 24 pages. Adding pages to your passport will not change its validity. Therefore, the second option is a better one if your passport only has a year or two before it expires and you do not think you will need all the added pages for visas and entry stamps. Two, you can apply for a renewal by mail. You will be issued a new passport with a validity of 10 years. If you plan to travel extensively over the next 10 years, you can check the box on the application form to receive the 52-page passport book rather than the one with only 28 pages. In addition to the completed Form DS-82, you need to send your current passport, a recent passport photo and payment in the amount of $110 for the renewal application fee. If your name has been legally changed, you also need to include evidence of this. Note, you cannot apply for added pages or a renewal by mail if your passport is significantly damaged. In this case, you need to appear at an application acceptance facility in order to apply for a new passport. Applications for both added pages and renewals are processed in 4-6 weeks for routine service. You can request expedited service by mail to get the process completed in 2-3 weeks. There is an additional fee of $60 to expedite either form. To have either application processed more quickly, you need to schedule an appointment at the nearest regional agency and either submit the application yourself or authorize a third party to do it for you. This can be a friend, relative, travel agent or registered passport expediting courier service.Source: http://www.us-passport-service-guide.com/passport-blog.html#What-to-Do-When-Your-Passport-is-Full
