What you will need to do to change your gender on your passport is have a signed original statement, on office letterhead, from a licensed physician who has treated you for gender-related care or has evaluated your gender-related medical history. All other requirements stand for whether you are getting a new passport or a renewal. If you have legally changed your name as part of the process, you must also submit for a name change.
